Aims and outcomes
Aim:
To improve patient safety and prevent communication breakdown when being transferred within the Trust.
Objectives:

To increase awareness of the transfer policy and its implications for practice

To raise awareness of patient safety prior to and during transfer

To develop awareness of signs of deterioration and recommended actions in this event
-
To improve communication skills therefore improving the patient experience
-
To be able to carry out basic life support
