Root cause analysis full (RCA)

Course description

This course is designed to equip delegates with the tools and knowledge required to conduct level one concise RCA investigations; or to participate in or to support a lead investigator carrying out a level two  comprehensive RCA investigation.

The course provides an in-depth look at key RCA tools, techniques and methodologies. Training is built around a case study and provides time to practise during group work. The course also introduces use of other processes involved during investigations such the incident decision tree and being open with patients and families.

Aims and outcomes
To equip delegates with the skills required to conduct a full root cause analysis in order to learn the appropriate lessons from adverse events, and to reduce the likelihood of similar events happening again.
Target audience

All staff responsible for supporting or conducting level one concise RCA investigations or for supporting a lead investigator carrying out a level two comprehensive RCA investigation.

Staff should have responsibilities for supporting or conducting RCA investigations in their workplaces.
