Hospital to home transfer
Information for patients who are being transferred form hospital to home.

You may return to Jersey by boat as an alternative to flying, as there are fewer restrictions on boat travel.

The Liaison Sister will book the flight/boat trip and the taxi for the journey to the airport/port. (Please ensure that if any assistance is required, for example, lift on and off the aircraft, wheelchair assistance etc. that the Liaison Sister is made aware of this prior to booking the flight/boat trip).

The Liaison Team will organise a taxi to the airport/ferry port for you and any relative/friend that may be accompanying you.

If an ambulance is deemed necessary to take you to the airport/port, please note that the ward is responsible for this booking. You will then be supplied with your travel details.

During out of hours, weekends and bank holidays when no Liaison Service is available to return you to Jersey on a scheduled flight/boat journey, the Senior Nurse will be able to repatriate you back to Jersey.

If an ambulance is required to take you back to the airport/port, the ward you are on will book this for you.
