We have made considerable changes to our discharge summaries over the past few months and more improvements are on their way.
Some of the things we've improved so far:

We've introduced a short version for day cases in surgery and gynaecology and are planning to implement these in a range of other departments by the end of the year.

We've introduced electronic summaries which are now delivered to the Hampshire Repository on the day of the patient's discharge. GPs can access the summaries directly at the Repository or, if we have your practice nhs.net emails, we can send them direct to you.

We have changed the content and format to provide greater clarity.
