Community / Home Care
Our palliative care sisters and other members of the multi-professional team, including palliative care consultants, work in the community with GPs and district nurses, to support patients and families in their own homes.
You may be referred to this service by your GP while you are at home or this may have been arranged before you were discharged from Countess Mountbatten House or other hospitals.
Community palliative care sisters are specialist nurses who visit you at home according to your needs and wishes, to offer support and advice on different aspects of your illness, treatment and medication. If specialist advice or help is needed from our physiotherapist, doctor, occupational therapist or social worker, this can be arranged. Other services that may be accessed via the team are volunteers, day care and complementary therapists. The community palliative care team works closely with your GP and district nurses to ensure your best possible support and care.
Each professional within the Team is responsible for a geographical area within Southampton, Winchester and Portsmouth health districts.
