Clinical nurse specialist – adults with congenital heart disease

“As a clinical nurse specialist for adult patients born with congenital heart disease my role involves providing a point of continued contact for adult congenital heart patients and families throughout their lives.  Congenital heart disease is considered a chronic illness, and people born with congenital heart disease can have chronic health issues that impact intermittently throughout their lives. My role incorporates the provision of patient information and education, the troubleshooting of clinical patient concerns, accessing appropriate care and providing ongoing support for patients and families. 

Patients can contact this service from the community, hospital clinics or during an inpatient stay through via a help line/ email service.”
