How to self-refer for maternity care

It’s easy to self-refer for maternity care at University Hospital Southampton NHS Foundation Trust, and you don’t need to go through your GP to do so.

Please refer to us when you are more than six weeks pregnant.

You can complete our maternity referral form to inform us about your pregnancy.

NHS number: Please have your NHS number ready to complete the referral. If you don’t know it, your GP surgery can provide it or you can visit the NHS website to find it here.

Providing information: Share as much detail as possible, as this helps us deliver the most appropriate care for you.

After submitting your self-referral

  • You will receive a confirmation email to acknowledge receipt.
  • Your GP will be notified that we are providing your care, as required.

You can expect to hear from the self-referral team within four weeks of submitting your form. In some cases, you may receive a response sooner depending on your gestation and clinical details. Once your referral is processed, you’ll receive details of your midwife booking appointment and instructions on how to log in to your BadgerNotes app.
