Making sure we can contact you
You will be aware of the growing number of cases of COVID-19 infection (commonly known as coronavirus) in the UK and worldwide. In the coming weeks the Trust may decide to convert some outpatient appointments to telephone rather than requiring patients to come to the hospital for a face to face appointment. As such, it is important that we have your latest contact details so we can inform you of any changes to your appointment(s) and/or conduct your appointment(s) over the telephone.
We often send text message and voicemail reminders of appointments. If you have received a letter from us at your current address, or have received text messages or voicemails from us about your appointment to your current numbers, you can be assured that we have your correct details. If any of these details have changed since your last contact with us they may need changing on our systems. If this is the case, please complete the form below.
Using this form
Notifying us when you've changed your address is also for your own protection. Any mail going to your old home puts you at risk of identity fraud.You must only submit this form once you've changed your address and not before. Details can be completed by a patient, parent or guardian.
You must complete one form per patient, not one per address. We won't send you confirmation when your details have been updated but all correspondence for future appointments will be sent out to your new address. Please complete all sections of the form that apply to you or we'll be unable to update your details
