Discharge Summary Timeliness from SUHT to GP 2008
The objective of this audit is to improve the timeliness of GP practices receiving discharge summaries for inpatients being discharged from Southampton University Hospitals NHS Trust.

Each GP Practice is being invited to submit up to 10 cases.  

SAMPLE:  The first 10 consecutive discharged summaries received starting 14th July 2008.  

Starting 14th July 2008, please complete the information in this audit tool, and submit ONCE for each discharge summary you receive up to a maximum of 10.   The deadline for submission of this data is 31st July 2008.
