Portsmouth Hospitals Healthy Lifestyle at Work Survey 2008
If you choose to participate, please complete and submit your response by 30th September 2008.
The Trust is keen to promote the health of its staff.  A fit and healthy workforce is key to success and will support the goals of the Trust.  This survey is intended to help the Trust to be a healthier place to work and to support a positive experience for patients who come to us for their healthcare.
The survey is entirely confidential - you will not be personally identified in any way.     We hope you will participate and help the Trust to:

-Learn more about the health of its staff
-Learn how their working day impacts on their health
-Develop an understanding of what is important to staff in terms of improving their health
-Identify how  the Trust can support them in doing this.

Your feedback will inform the Trust about what is happening now and how you can be supported in achieving a healthier lifestyle while at work.  Then, we can start working together to make it happen!
