You have a right to see the information we hold about you. In this section we explain how you can request that information from us.

View your medical record online

The easiest and quickest way to access your medical record is to register with our My medical record service - this gives you access to lots of medical information, appointments and letters that the Trust holds about you. You can register for this service here.

What types of information do we hold?

We hold different types of information about patients:

• Medical records which includes notes, details of visits and doctors opinions
• Images which includes things like x rays and scans

This information is kept on two different systems so you will need to be clear what type of information you would like and make the request to the appropriate team.
