Our patient support hub offers assistance to patients, their families and unpaid carers. Our volunteers can support you during a visit to hospital and at home, before, during and after discharge. We can also refer you to other community-based voluntary service providers who can help you depending on your individual needs.

The patient support hub was developed to be a single point of contact for patients and their families who need extra support from UHS. Our team of volunteers provide practical and emotional support to patients.

We complete surveys on behalf of the Trust for specialist teams and will contact you after discharge to follow up and make sure that you have everything that you need.

Our volunteer call handlers will guide patients through the process of arranging the help and support that they need to ensure their visit to UHS goes smoothly.
