Involved patients

To become an involved patient, you have to have been seen at University Hospital Southampton in the last 12 months and you can fill out an application using this form.

Your application will tell us all we need to get you signed up; it asks about the areas of the hospital you have experience with and your demographic details so we can tailor opportunities to you when more specific requests for feedback come our way. Your details will only be shared with other hospital staff if you have signed up for an opportunity that they are working on and the feedback that you share with us will not affect your future care.

Opportunities will be emailed to you (a bit like a mailing list) and we try to never send more than one opportunity a week. Emails will include a link either to a survey or to a form to register your interest in any in-person or virtual events and if an opportunity isn’t relevant to you, you can ignore that email and come back for the next opportunity.

Every email we send has an unsubscribe link at the end which will take you to a page where you can archive your account and stop receiving emails. Alternatively, we can manually archive your account if you respond to one of our emails requesting us to remove you.

Any previous feedback you have shared with us will be kept as this is needed for the projects you have supported on to evidence patient involvement, but the majority of our surveys are anonymous. When we do ask you to identify yourself (for in-person or virtual events for example) it is always your choice and never mandatory. All minutes from focus groups and workshops are written by the patient engagement and involvement officer who will use initials to identify what people have said rather than identifiable names.
