Hampshire and Isle of Wight Sustainability and Transformation Plan (STP)

We have been working with health and care organisations across Hampshire and the Isle of Wight to agree on how best to meet the many opportunities and challenges facing the local health and care system. This has focused on both the need to empower people to stay well and also on how we can provide safe, high quality, consistent and affordable health and care to everyone.

This does not replace or slow down our local transformation programme. Instead, we have come together to do the things that can only be achieved by working together across the whole of Hampshire and the Isle of Wight. We have learned a great deal from working with and listening to local people over the past few years and this plan is rooted in these local discussions. Working together also allows us to share better our best practice and ensure we are coordinated when we make local changes. 

The challenges we face are not unique to our area. In fact, Hampshire and the Isle of Wight is one of 44 areas across England that are developing detailed local sustainability and transformation plans (STPs) to find ways of solving them.

Hampshire and Isle of Wight Local Digital Roadmap

The Local Digital Roadmaps (LDR) programme aims to shape national policy and strategic investment through a detailed understanding of local health and care systems’ plans to use digital technology to drive sustainability and transformation. LDRs, aligned to Sustainability and Transformation Plans (STP), will provide the evidence base and justification to underpin new investment and increased optimisation of existing assets.
