Change or update your address

It is important that we have your latest contact details so we can inform you of any changes to your appointment(s) and/or conduct your appointment(s) over the telephone.

We often send text message and voicemail reminders of appointments. If you have received a letter from us at your current address, or have received text messages or voicemails from us about your appointment to your current numbers, you can be assured that we have your correct details. If any of these details have changed since your last contact with us they may need changing on our systems.

Changing your address

To change your address, you must be a UHS registered patient.

If you are a UHS patient, to request to change your address please email PatientServicesCentre@uhs.nhs.uk. Please use the following headings in the email:

  • Email subject/heading: Patient request to change address
  • Email body: Please provide:
    • NHS/Hospital number (required):
    • Full name (required):
    • Date of birth (required):
    • Current address (required):
    • New address (required):

Once your address has been changed, we will confirm this with you by email.
