Critical care outreach team (CCOT)

Who are we?

We are an expert team of advanced practitioners with extensive critical care, emergency department or emergency medical care experience. We provide critical and acute care outside of the intensive care unit to those who need it.

We are on call 24 hours a day, seven days a week and can be contacted using Bleep 9191 or 9172 or can be called through switchboard (100).

We support all unwell adults across Southampton General and Princess Anne Hospital. Our team are trained as advanced practitioners and have the skills to diagnose, make decisions regarding treatment and prescribe medications to tackle life threatening conditions such as sepsis as quickly as possible. This helps avoid admissions to critical care by stabilising and managing very unwell adults on the wards. However, we can also help to speed up the process of being admitted to critical care if needed.

CCOT has four main priorities:

  1. Ensuring that critically ill patients or those at risk of becoming so, receive appropriate and timely investigation, treatment and intervention in the most suitable location within the hospital environment.
  2. Providing support for ward staff in responding to and supporting patients who are discharged from an intensive care unit (ICU) or high dependency unit (HDU) to a ward area following a period of critical illness.   3. Delivering educational support around ‘the deteriorating patient’ to the multidisciplinary workforce in the form of face-to face teaching sessions, simulation and bedside teaching.
  4. Providing a 24/7 response to all adult emergency calls across the Trust

Call 4 Concern

Call 4 Concern© (C4C) is an inpatient safety service. It provides patients and families with a simple process with which to activate a clinical review, if they are concerned that either their own clinical condition, or that of their relative is deteriorating and their worries and concerns have not been addressed by the ward-based team.

Call 4 Concern is a dedicated 24/7 rapid response service that is triaged by the UHS site operations team. The Critical Care Outreach Team will review all the Call 4 Concern calls that are related to a clinical deterioration.
