Referring hospitals

There are some key data items required to report transplant patient activity to the EBMT data registry. We need your support, as a referring centre, to be able to do this and maintain transplant services provided by Southampton

This data is required for EBMT membership registration and it is essential for JACIE accreditation. It is also now become a national requirement by commissioners to inform funding and practice of services provided by transplant centres in the UK using the annual BSBMT outcome activity report.

We have created specific forms to enable you to provide the data required - see below for transplant patient data requirements for EBMT.

Transplant patient data reporting requirements

Please provide the following information at diagnosis and follow up:

  • Initial diagnosis

At time of referral for transplant please provide us with:

  1. referral letter
  2. completed patient referral initial diagnosis form - select the appropriate disease indication form
  3. diagnostic material (if available)
  • Follow-up - 100 days and annually

For transplant patients that you see for follow up please provide:

  1.  completed follow up form - select either:
  • 100 day follow up form
  • annual follow up form

2. copies of any results

3. copy of the clinic letter
